You will need Adobe Acrobat Reader installed on your computer to view and print from these templates. Keep in mind that you won't be able to save your work unless you have the full version of Adobe Acrobat.

Select one of the forms above, and click the link to download and open the Adobe Acrobat file. Most files are password protected, so you will need to enter the same password you use for access to the Lenders Matrix and list of PLF Appraisers. If you've forgotten your password, contact Ron or Lewis.

Text fields in both the Uniform Residential Loan Application and the Mortgage Loan Disclosure Statement can be edited in Adobe Acrobat Reader (must have full version to save changes). All other forms can be printed and completed.
